Parental Leave

As part of Miami's effort to foster a successful work/life balance, benefit-eligible employees are provided with paid parental leave to be used following the birth or adoption of a child. See MUPIM 4.6/OAC 3339-4-06 for policy details.

How to Apply

  1. Complete and submit the Request for Parental Leave form.
  2. Once submitted, eligibility will be verified and request for medical certification faxed to your physician to complete and return by secured fax. 
  3. After medical certification has been submitted notification will be sent via your Miami email and United States Postal Service within five (5) business days (unless the University is closed).
  4. Your physician has fifteen (15) days to return your completed medical certification. 
  5. From the time you submit your FMLA Request, you have fifteen (15) days to be in FMLA pending status.
