Orientation

Once you have accepted your offer of employment (and completed the online personal data sheet), you will receive an email from Benefits and Wellness, inviting you to a new hire orientation session. We conduct new hire orientation sessions on Mondays from 1:30–3:30 p.m. at the Staff Development Center (424 Wells Mill Drive, Oxford, Ohio 45056). Should Monday be a paid holiday, the session will be conducted on Tuesday.

At your orientation session, you will complete all necessary forms for payroll and benefits (if eligible), including:

  • Form SSA-1945 (This form deals with employment in a job not covered by Social Security (see our Retirement page for details about state retirement plans) and you must complete it during new hire orientation. We provide a copy of the statement in your new hire packet.)
  • Tax form W-4
  • Payroll direct deposit enrollment (You will set this up in BannerWeb)

We will also introduce miscellaneous University policies and procedures. You will sign receipt of the following policies:

  • Harassment policies (MUPIM 3.6/OAC 3339-3-06)
  • Ethics policies (MUPIM 3.11/OAC 3339-3-11)
  • All employees hired into an American Federation of State, County, and Municipal Employees (AFSCME) position will receive at orientation a copy of the current agreement (see also AFSCME and FOP Agreements to review this contract in advance)

What to Bring to Orientation

You will need to bring your banking information (account and routing numbers) to set up direct deposit of your paycheck.

If you are benefit eligible and are going to cover a spouse or dependents, you must bring copies of your marriage certificate and child(ren)'s birth certificate(s). If you know that you plan to cover your spouse on your health plan, you may complete the Spouse Health Coverage form (for health coverage only), allowing Miami to verify your spouse's eligibility for Miami's coverage.

Please note: You have 31 days from hire to submit the dependent verification documentation to the Benefits office. Dependents will be dropped from coverage at the end of the month after the 31-day deadline, and will not be able to be added until open enrollment in October for the following year.
