Adding a Google Form

Create Form in Google Drive:

A. Click on New and then More >> Google Forms

B. Make sure that you UNCHECK the first box so that users are not required to log in.

Question Type: Text —User can input a couple of words.

D. Type in your closing instructions or comments. Check boxes as appropriate.
    Click Send Form to open the dialogue box.

E. Click Embed.

Manage Form Responses:

Google Forms will automatically create your response spreadsheet. You will see both the form and the form (responses) in your Google Drive menu.

Set Notification Rules:

A. Open your Form(Responses) spreadsheet. 

You can open it from your Google Docs menu (as shown above) or from within your form (as shown below).

B. Click Tools, then Notification Rules.

C. Choose the options that meet your needs. The image shows the most common settings.
