Field Entry
Query Date Employee's first day of work
Approval Category Unclassified Exempt, UNCEXT
If an employee makes less than $455 per week, choose UNCLASSIFIED NON-EXEMPT, UNCNEX.
Position and Suffix Select Position/Suffix from All Jobs list that matches the appointment to which the person is being reappointed.
Jobs Effective Date Employee's first day of work
Personnel Date Employee's first day of work (Should match Jobs Effective date)
FTE and Hours per Pay If either value changes, please contact Taylor Kwalton for new calculated values.
End Job Record Dates JOBS EFFECTIVE DATE: Employee’s last day of work
PERSONNEL DATE: Employee’s last day of work (Should match Jobs Effective date)
Routing Queue Level 10 Academic Personnel Reviewer: Betsey Dietrich (masonej)
Level 45 Chair/Supervisor: Choose appropriate chair or director
Level 50 Dean: Choose appropriate Dean Designee (NOT the actual dean)

Comment Field

Percent time of appointment is related to the number of hours the employee works per week. For example, an employee working 20 hours per week is 50% time. If the position is grant funded or externally funded, add the phrase “Vacation must be taken prior to the end date of this appointment. Any vacation balance becomes zero at the end of appointment."

Add a semi-colon at the end of each line of text in the comment field.

Do not put apostrophes in the comment field.
