Change of Schedule Form

The Change of Schedule Form [PDF]  is used, in accordance with university regulations and deadlines, to add or drop a course or to change the status of a course from grade to credit/no credit or audit (or vice versa). In all cases, the signature of the instructor (or departmental designee if instructor is not available) is required. The form must then be submitted to the One Stop in CAB for processing.

Permission for Excess Hours

The Permission for Excess Hours form is used to request permission to take a course load in excess of 20 hours in Fall or Spring semester (lower limits are in effect for Summer (16) and Winter terms (6)). A student must secure support and signature from their academic or department advisor and then the divisional advisor prior to submitting the form. The Excess Hours form will be available in the department or divisional advising offices.

Petition to Modify Miami Plan Foundation Requirements & Self-Designed Thematic Sequence Proposal Form

Petitions to modify the Miami Plan or propose a Self-Designed Thematic Sequence or Capstone can be submitted online.

Withdrawal from the University

The Withdrawal from the University [PDF] form is used to withdraw from all coursework for the semester or term. A student must obtain the signature of the divisional advisor prior to submitting the form to the One Stop in CAB for processing. A student who withdraws for the semester will need to re-enroll and in some cases, petition to be permitted to re-enroll to return to the university.

EHS Undergraduate Student Petition

The Undergraduate Student Petition [PDF] is used to request an exception to a departmental, divisional (EHS) or University policy. A student must first meet with their academic advisor before submitting a petition.

EHS Substitution or Waiver of a Course

The EHS Substitution or Waiver of a Course [PDF] is to be used by EHS departments when requesting changes or adding comments to a Student DAR. A student must first meet with their academic advisor before submitting a request.

EHS Request for Course Drop or Withdrawal After the Deadline

The Request for Course Drop or Withdrawal [PDF] form is for students seeking to drop a course after the drop deadline but within the current term OR to withdraw from a course after the withdrawal deadline but within the current academic year (fall, winter, spring, summer) and prior to September 1st must petition through the Interdivisional Committee of Advisers (ID). Students are reminded that the Miami University withdrawal policy acknowledges that under extenuating circumstances (defined as documented extraordinary circumstances outside of a student’s control) students may request to withdraw from a course after the 60% deadline. Students are advised to continue attending the class(es) from which they are requesting to withdraw until they receive a decision from the ID Committee. A student must first meet with a department or divisional advisor before submitting a petition.
