Healthy Miami Premium Discount Program

We provide the Healthy Miami Premium Discount Program to all benefit-eligible employees and their spouses covered under Miami's health plan.

By participating in this voluntary program and completing specific healthy steps, you can reduce the cost of your premium by up to $720 per calendar year.

Spouses covered under your health plan can also voluntarily participate in the program. Combined participation determines your total savings ($1440).

Program Steps

The steps you are required to complete in a calendar year to receive premium discounts depend on your age and gender.

Individuals that cannot complete the base steps may have their physician submit a Medical Exemption to qualify for a portion of the premium discount. Similarly, the Alternate Proof of Care form can be used if a step has been completed outside of the Miami health plan. All forms can be found in your Healthy Miami account. Please contact the Benefits office at 513-529-3926 if you have any questions regarding these forms.

Please note: We base our frequency of screenings on recommendations by the U.S. Preventive Services Task Force and the U.S. Department of Health and Human Services.

Getting Started

Complete the program steps between January 1 and November 15 in a calendar year and qualify for the premium discount for the following calendar year.

When You and/or Spouse Enroll in Miami's Health Insurance

The application of the discount varies based on when you and/or a spouse enroll in health coverage.

  • Enroll between January 1 and June 1
    • Receive discount for current year
    • Complete the program steps in the current year for the discount in the following year.
  • Enroll between July 1 and December 1
    • Receive discount for the remainder of the current year and the following year
    • Complete the program steps in the following year

How Much You Can Save

  • $180 per year: Complete required base steps for your age group without completing the Tobacco-Free step
  • $540 per year: Complete the Tobacco-Free Step only
  • $720 per year: Complete all required steps for your age group

If your spouse is covered under your health plan, they can also voluntarily participate in the program. Combined participation determines your total savings. For example, if you both complete all steps, you will actually save $1,440.

When you complete all of the Healthy Miami Steps, no additional amount will appear on your check as an additional amount to your medical premium.

Female Under Age 50

If you are 49 or younger in 2023, you must complete the following steps before November 15, 2023, in order to qualify for the premium discount in 2024.

Tobacco-Free Certification

Employees and spouses enrolled in Miami University's healthcare plan are eligible to receive a premium discount for being tobacco-free. In order to meet this required step and earn a point in your Healthy Miami account, you must do ONE of the following every year:

  1. Complete the Tobacco-Free Certification, which you can access in your Healthy Miami account OR
  2. Complete a university-approved tobacco cessation class OR
  3. Complete the tobacco cessation coaching program (via phone 800-227-6007) with Impact Solutions Employee Assistance Program, username: Miamioh, and submit the Proof of Tobacco Cessation Coaching Form.

Health Plan Provider Consent

By completing this form, you allow Anthem to send Cerner Wellness basic participation data about your completion of the Healthy Miami premium discount program. In order to meet this required step and earn a point in your Healthy Miami account, you must complete the Health Plan Provider Consent form, which you can access in your Healthy Miami account every year.

Please note: Anthem does not share any information pertaining to diagnosis, treatment, or results from the medical appointments. The only information that Anthem will share with Cerner Wellness and Miami is whether you have completed an activity associated with the Healthy Miami Premium Discount Program, such as having a Primary Care Physician (PCP) Wellness Visit.

Biometric Health Screening

This health screening, which you must complete every year, includes measurement of your blood pressure, and a fasting blood draw. You should not eat or drink anything (except water) at least 12 hours before your screening. We encourage drinking plenty of water, and you should take any daily prescription medications as your doctor has prescribed.

You can complete your screening in one of two following ways.

University-sponsored Option

In order to ensure the safety of Miami employees, please review the enhanced safety measures related to the Biometric Screening process.

You may schedule an on-campus screening at no cost. To use this option, register for a biometric health screening by logging into your Healthy Miami account, scrolling to the "Upcoming Events" section, and selecting the campus of your choice. You will need to bring the following to your scheduled screening:

  • A valid ID card (e.g., university ID for employees and Anthem ID for spouses)
  • The completed Consent for Biometric Health Screening form from TriHealth
  • Your physician's name and address (you must have a PCP in order to complete the screening)

University-sponsored option includes the following measurements:

  • Blood pressure (mmHg)
  • Fasting blood draw
  • Comprehensive Metabolic Panel (CMP): albumin, alkaline phosphatase, ALT, AST, total bilirubin, calcium, carbon dioxide, chloride, creatinine, glucose, potassium, protein, sodium, urea nitrogen (BUN)
  • Lipid panel: cholesterol, HDL, LDL, triglycerides, chol/HDL ratio, LDL/HDL ratio-calculated
  • Iron
  • Hemoglobin A1C (if fasting glucose is over 100)
  • Complete Blood Count (CBC): white blood cells, red blood cells, hemoglobin hematocrit mvc, mch, mchc, rdw, platelet count
  • Thyroid Specific Hormone (TSH)
  • Colon Health Kit (take home, if applicable)

Please note: We recommend completing your health screening prior to your annual PCP visit. The results will be sent to your physician within 48 hours of your health screening.

Alternative Option

If you choose not to use the university-sponsored biometric health screening, you may meet this step by having a physician or facility of your choice complete your fasting blood draw. Please note: This option may mean physician, lab, or other charges.

To use this option, you must do the following:

  1. Schedule your appointment.
  2. Once you receive the results from your physician, complete the Biometric Health Screening Form (found in your Healthy Miami Account), attach a copy of your results, and submit it to TriHealth.

Primary Care Physician Wellness Visit

A Primary Care Physician (PCP) is a general practitioner, family physician, or an internist that serves as your primary healthcare professional. Your PCP handles a variety of health-related problems and keeps your medical history and medical records.

To meet this step, you must complete a wellness visit with your PCP every year. Miami's health plan covers a wellness visit at 100% once per calendar year. You do not need to wait 365+ days.

Your wellness visit includes:

  • a physical exam
  • review of your biometric health screening results
  • discussion of your overall health and well-being
  • preventive cancer screenings

Please note: Women who receive their cervical cancer screening (PAP test) with an OBGYN will have two physician (PCP and OBGYN) visits per year. Each visit is covered 100% when the claim is submitted as preventive.

Special Circumstances

If you completed a primary care physician wellness visit within the required time frame before being covered under Miami's Anthem plan, or the claim was not filed with Anthem because Anthem was your secondary coverage, or you used a provider that does not file claims, you must submit a copy of the documentation to TriHealth using the Proof of Alternate Care Form (found in your Healthy Miami Account) by the November 15 deadline.

Preventive Screening: Cervical Cancer

To complete this step, you must have a physician perform a routine PAP test every three years.

Your should receive credit for your PAP test for three years in your Healthy Miami account—it should post automatically after you have completed the screening for each year during the three-year time frame. If your point has not automatically been uploaded, please contact the Cerner Wellness team (888-252-8150).

Please note: Women who receive their cervical cancer screening (PAP test) with an OBGYN will have two physician (PCP and OBGYN) visits per year. Each visit is covered 100% as both are preventive.

Special Circumstances

If you completed a PAP test within the required time frame before being covered under Miami's Anthem plan, or the claim was not filed with Anthem because Anthem was your secondary coverage, you must submit a copy of the documentation to TriHealth using the Proof of Alternate Care Form (found in your Healthy Miami Account) by the November 15 deadline.


Earning Points in your Healthy Miami account

For each step you complete, you will earn one point in your Healthy Miami account (typically, it takes 4 - 6 weeks for points to post).

Female Age 50 and Over

If you turn 50 or older in 2023, you must complete the following steps before November 15, 2023, in order to qualify for the premium discount in 2024.

Tobacco-Free Certification

Employees and spouses enrolled in Miami University's healthcare plan are eligible to receive a premium discount for being tobacco-free. In order to meet this required step and earn a point in your Healthy Miami account, you must do ONE of the following every year:

  1. Complete the Tobacco-Free Certification, which you can access in your Healthy Miami account OR
  2. Complete a university-approved tobacco cessation class OR

Personal Health Assessment

To meet this step every year, you must log in to your Healthy Miami account and complete the online personal health assessment (under the "My Personal Health Assessment" tab). This assessment, which consists of a series of health behavior questions, will provide you with a snapshot of your health status, an evaluation of your health risks, and feedback on steps that can be taken to improve your health.

Please note: Answers to questions in the personal health assessment are private and confidential, according to HIPAA guidelines. You can change your health assessment answers at any time if there is a change in your health status.

Biometric Health Screening

You complete this step by receiving a routine mammogram every two years. Miami's health plan covers this screening—there is no additional cost to you. You may choose from any of the following ways of scheduling your screening:

  1. Have your PCP place the order for your mammogram
  2. Schedule on your own
  3. Visit the Mobile Mammography van when it is on campus

Special Circumstances

If you completed a mammogram within the required time frame before being covered under Miami's Anthem plan, or the claim was not filed with Anthem because Anthem was your secondary coverage, you must submit a copy of the documentation to TriHealth using the Proof of Alternate Care Form (found in your Healthy Miami Account) by the November 15 deadline.

Preventive Screening: Cervical Cancer

Preventive Screening: Colon Cancer

You may complete this screening in one of three following ways: having a colonoscopy every ten years, completing a colon health kit every year, or completing the Cologuard test every three years.

Colonoscopy (Every Ten Years)

To schedule your colonoscopy, ask your physician to place an order for it. You may have this screening done at an in-network facility of your choice. Certain colon prep kits are covered at 100% under the Miami health plan.

Special Circumstances

If you completed a a colonoscopy within the required time frame before being covered under Miami's Anthem plan, or the claim was not filed with Anthem because Anthem was your secondary coverage, you must submit a copy of documentation to TriHealth using the Proof of Alternate Care Form (found in your Healthy Miami Account) by November 15.

If you had your colonoscopy within the required 10-year time frame and received credit for a previous year, your point will automatically upload for the current year until the 10 years has expired and you are due for another. If your point has not automatically been uploaded, please contact the Cerner Wellness team at 1-888-252-8150.

Colon Health Kit (Every Year)

If you choose to complete a colon health kit every year, you may pick one up at any of the following locations:

  • Healthy Miami-sponsored screening
  • The Employee Clinic on the Oxford Campus
  • Your physician's office

This kit can be completed in one day by following the instructions and submitting the completed kit.

Please note: If you complete a kit through your physician's office, you must have your physician complete the Biometric Health Screening Form (found in your Healthy Miami Account) with the results and submit the form to TriHealth.

Cologuard (Every 3 Years)

You may choose to complete Cologuard once every 3 years, but you must have a prescription from a physician. Cologuard is covered at 100% through our health plan.


Earning Points in your Healthy Miami account

For each step you complete, you will earn one point in your Healthy Miami account (typically, it takes 4 - 6 weeks for points to post).

Male Under Age 50

Preventive Screening: Colon Cancer

You may complete this screening in one of three following ways: having a colonoscopy every ten years, completing a colon health kit every year, or completing the Cologuard test every three years.

Colonoscopy (Every Ten Years)

To schedule your colonoscopy, ask your physician to place an order for it. You may have this screening done at an in-network facility of your choice. Certain colon prep kits are covered at 100% under the Miami health plan.

Special Circumstances

If you completed a a colonoscopy within the required time frame before being covered under Miami's Anthem plan, or the claim was not filed with Anthem because Anthem was your secondary coverage, you must submit a copy of documentation to TriHealth using the Proof of Alternate Care Form (found in your Healthy Miami Account) by November 15.

If you had your colonoscopy within the required 10-year time frame and received credit for a previous year, your point will automatically upload for the current year until the 10 years has expired and you are due for another. If your point has not automatically been uploaded, please contact the Cerner Wellness team at 1-888-252-8150.

Colon Health Kit (Every Year)

If you choose to complete a colon health kit every year, you may pick one up at any of the following locations:

  • Healthy Miami-sponsored screening
  • The Employee Clinic on the Oxford Campus
  • Your physician's office

This kit can be completed in one day by following the instructions and submit the completed kit.

Please note: If you complete a kit through your physician's office, you must have your physician complete the Biometric Health Screening Form (found in your Healthy Miami Account) with the results and submit the form to TriHealth.

Cologuard (Every 3 Years)

You may choose to complete Cologuard once every 3 years, but you must have a prescription from a physician. Cologuard is covered at 100% thorugh our health plan.

Please note: Answers to questions in the personal health assessment are private and confidential, according to HIPAA guidelines. You can change your health assessment answers at any time if there is a change in your health status.

  2. Once you receive the results from your physician, complete the Biometric Health Screening Form (found in your Healthy Miami Account), attach a copy of your results, and submit it to TriHealth.

Primary Care Physician Wellness Visit

A Primary Care Physician (PCP) is a general practitioner, family physician, or an internist that serves as your primary healthcare professional. Your PCP handles a variety of health-related problems and keeps your medical history and medical records.

To meet this step, you must complete a wellness visit with your PCP every year. Miami's health plan covers a wellness visit at 100% once per calendar year. You do not need to wait 365+ days.

Your wellness visit includes:

  • a physical exam
  • review of your biometric health screening results
  • discussion of your overall health and well-being
  • preventive cancer screenings

Special Circumstances

If you completed a primary care physician wellness visit within the required time frame before being covered under Miami's Anthem plan, or the claim was not filed with Anthem because Anthem was your secondary coverage, or you used a provider that does not file claims, you must submit a copy of the documentation to TriHealth using the Proof of Alternate Care Form (found in your Healthy Miami Account) by the November 15 deadline.

Preventive Screening: Colon Cancer

You may complete this screening in one of three following ways: having a colonoscopy every ten years, completing a colon health kit every year, or completing the Cologuard test every three years.

Colonoscopy (Every Ten Years)

To schedule your colonoscopy, ask your physician to place an order for it. You may have this screening done at an in-network facility of your choice. Certain colon prep kits are covered at 100% under the Miami health plan.

Special Circumstances

If you completed a a colonoscopy within the required time frame before being covered under Miami's Anthem plan, or the claim was not filed with Anthem because Anthem was your secondary coverage, you must submit a copy of documentation to TriHealth using the Proof of Alternate Care Form (found in your Healthy Miami Account) by November 15.

If you had your colonoscopy within the required 10-year time frame and received credit for a previous year, your point will automatically upload for the current year until the 10 years has expired and you are due for another. If your point has not automatically been uploaded, please contact the Cerner Wellness team at 1-888-252-8150.

Colon Health Kit (Every Year)

If you choose to complete a colon health kit every year, you may pick one up at any of the following locations:

  • Healthy Miami-sponsored screening
  • The Employee Clinic on the Oxford Campus
  • Your physician's office

This kit can be completed in one day by following the instructions and submit the completed kit.

Please note: If you complete a kit through your physician's office, you must have your physician complete the Biometric Health Screening Form (found in your Healthy Miami Account) with the results and submit the form to TriHealth.

Cologuard (Every 3 Years)

You may choose to complete Cologuard once every 3 years, but you must have a prescription from a physician. Cologuard is covered at 100% thorugh our health plan.


Earning Points in your Healthy Miami account

For each step you complete, you will earn one point in your Healthy Miami account (typically, it takes 4 - 6 weeks for points to post).
