Complete and submit the waiver prior to your appointment at the Police Services Center.
Complete waiver and payment and report to the Miami University Police Department (located next to Ditmer Parking Lot at 4945 Oxford Trenton Road / across from the horse stables) to be fingerprinted. Make sure to bring a valid driver's license with you. If you do not have a driver's license refer to “What to bring” for guidance.
This portion is completed once you arrive at the Miami University Police Department to be electronically fingerprinted.
This portion is completed only if a department or business is paying for the background check – leave blank if the individual is paying.
Special Note: Please do not attempt to email the form containing your SSN.
Results are mailed from London, Ohio to the address you provide at the time of service and typically take 7-10 business days. Results may be picked up in person at the Miami University Police Department and typically take 3-5 business days. Please note that on occasion results may be delayed and could take up to 30 days to receive.
If planning to pay by e check or credit card, the online payment link will be provided to customers prior to their scheduled appointment.
Bring a valid driver's license. If you do not have a driver's license, we will need you to bring one of the following along with a current photo ID: a passport, a state ID card, or a birth certificate. If you are under the age of 18, a parent consent form must be filled out before we can process the background check, please call to make arrangements.
Miami University Police Department does not retain copies of the results from background checks.
The FBI check can only be conducted when state legislation has been passed requiring the FBI check. The legislation must be approved by the FBI. BCI cannot process FBI background checks for individuals seeking employment, licensing, or volunteering in other states. If the reason for the background check request is not on the list of FBI reason codes on the Web Check workstation, a background check (black ink card) must be submitted to the FBI under the Freedom of Information Act.
