Financial Aid Forms: Information and Login

Required Forms

Current Miami University students can view their required forms on the Home tab in Banner 9 Self Service - Financial Aid. Missing requirements will delay payment of financial aid and could disrupt your enrollment. For this reason, it is important to complete these requirements as soon as possible.

Students can complete and review the status of their submitted forms in their Financial Aid Dashboard’s Requirements to Finalize Aid section.

If you are a dependent student, some forms may require your parent or guardian’s signature. Once you add your parent or guardian as an authorized user, they will receive an email with login information to the portal. Authorized users can access their student’s required forms on the Parent Financial Aid Dashboard. 

At the bottom of this page, you can find navigational videos on finalizing your financial aid, completing financial aid forms, adding an authorized user, and parent signatures. 

Student-Initiated Forms

The forms in the below tables are optional student-initiated forms, such as appeals and applications. If applicable, students and parents can visit StudentAid.gov to complete entrance counseling, master promissory note (MPN), and apply for a PLUS loan.
