Online Incident Report Form

Title IX of the Education Amendments 1972 (20 U.S.C. §1681 et seq) and its implementing regulations, "No person in the United States, shall, on the basis of sex, be excluded from participation in, be denied the benefits of, or be subjected to discrimination under any education program or activity receiving Federal financial assistance." Title IX of the Education Amendments of 1972, and its implementing regulation at 34 C.F.R. Part 106 (Title IX).

Any student, employee, or applicant for employment or admission to Miami University who believes that they have been discriminated against on the basis or sex may file a complaint with the Title IX Coordinator. To file a complaint against a Miami University student, faculty, staff or visitor for sex discrimination or any form of sexual misconduct including harassment, stalking, or assault, contact the Title IX coordinator or any deputy Title IX coordinators. A report can be submitted by mail, by email, in person, by telephone, or by using the electronic incident reporting form. Reports may be made at any time, even after regular business hours. Upon receipt a report, the Title IX Coordinator/Deputy Title IX Coordinator will assist the complainant in identifying the appropriate Miami University policy and corresponding grievance procedure to resolve the complaint in a prompt and equitable manner.

The University’s process for addressing Prohibited Conduct are grounded in fairness and support for all parties, include procedural protections that ensure notice and meaningful opportunities to participate, and recognize the dynamics involved in Prohibited Conduct.

Inquiries about the application of Title IX may be referred to the Title IX Coordinator, the U.S. Department of Education’s Assistant Secretary for Civil Rights (OCR) or both.
