Miami University campus name tags are produced by Plastigraphics. Please fill out the request form below. Once completed you will be provided with a digital proof via email from Plastigraphics. All proofs need approval before proceeding with production. Typical turnaround time from request to delivery is 10-14 business days.
Name tag submissions are processed twice a month. If you have an urgent need please contact wissmas2@MiamiOH.edu.
Large Orders
If you have a large name tag order, please use the template linked below to fill in all the necessary information. After completing the template, submit one Google Form for the order and email the completed template to Sydney Wissmar at wissmas2@MiamiOH.edu. This information will then be sent to Plastigraphics.
