Family & Medical Leave (FMLA) and Certifications
The Family and Medical Leave Act (FMLA) provides eligible employees up to 12 workweeks of unpaid leave a year. Contact Academic Personnel Services to help determine if you should request FMLA. Often, accrued leave will be used to continue one's salary.
When an individual is going to be absent for more than 5 consecutive days for medical reasons, including pregnancy/birth, Miami requires both a request from the individual and a medical certification from the healthcare provider.
After receipt of a request, Academic Personnel Services may contact the employee to request additional information.

- Complete and submit the Academic Personnel FMLA request form
- Request Employee's Serious Health Condition Medical Certification–to be completed by the healthcare provider and delivered, faxed or mailed to Academic Personnel Services
- Request Family Member's Serious Health Condition Medical Certification–to be completed by the family member's healthcare provider and delivered, faxed, or mailed to Academic Personnel Services
