The IRS Form 1095-C is now available in BannerWeb Employee Services Online.

IRS Form 1095-C, “Employer-Provided Health Insurance Offer and Coverage,” is the document Miami University provides detailing any employer-based health insurance coverage you had in 2019.

If you are a former employee of Miami, or if you requested paper copies of your 1095-C, you will receive the document via U.S. Mail at the W-2 address that is on file in Payroll.
