Dependents Covered Under Miami Benefits
- If covered by voluntary life that you elected, contact the Benefits office at 513-529-3926 to begin the claim process.
- If covered under health, dental, vision, life or accidental death insurance, you can update coverage in Workday.

Continue to fill in information and submit as prompted. You will be taken to a page that allows you to edit your Benefits. Once you submit this, you will have done all you need to do.

Not to Miami. If you had life insurance coverage for the dependent, once the Benefits office submits the claim, you will be contacted by Liberty Mutual to submit an original death certificate to them.
