Change of Program Forms

A faculty or professional advisor may submit a request to change an element of a student's listed academic program (change major, add/remove major, minor, or thematic sequence). The request is initiated by completing a “Change of Program Form” within the appropriate department link. Forms submitted by faculty and professional advisors are forwarded to the department designee, who makes the official change in native banner.
