Health Insurance

Miami offers two health plan options through Anthem. You choose the plan that is best for you and your family. Option 1 is the Traditional PPO and option 2, is the High Deductible Health Plan (HDHP) with a Health Savings Account. Both plans include pharmacy coverage.

Who is Eligible?

Staff appointed to a position working at least 30 hours per week and for more than one semester at .75 FTE are eligible for Miami's benefits.

Effective Date of Coverage

Beginning the first of the month following the first day of employment or eligibility, or immediately if the first day of eligibility is the first day of the month.

Who Can Be Covered

Dental and Vision Plans

For dental and vision plans, you can cover your legal spouse and/or your dependent children (through the end of the month in which the child turns 26 years old). You must submit required dependent documentation (see Qualifying (Family/Life) Events for details).

Medical Plan

Employees can cover a spouse on Miami's health plan if the spouse is:

  • unemployed.
  • self-employed and does not provide health insurance for anyone they employ.
  • self-employed and provides coverage, but pays more than 50% of the total premium.
  • retired and the only source of health coverage is Medicare.
  • eligible for retiree coverage, but required to pay more than 50% of the total premium.
  • employed, but not eligible for their employer’s coverage.
  • employed, but required to pay more than 50% of the total premium.

The Spouse Health Coverage form must be completed in order to cover a spouse on Miami's health plan. If spouse is employed or retired, the accompanying Employer Certification must also be completed and submitted.

You can cover your dependent children through the end of the month in which they turn 26 with required documentation.

 

Levels of Coverage

Single dental and vision coverage: you only

Family dental and vision coverage: you and a spouse and/or child(ren)

Single health coverage: you only

Employee Plus health coverage: you and spouse or you and child(ren)

Family health coverage: you, spouse, and child(ren)

Finding a Provider and Using Your Anthem Card

Find a Provider

Call Anthem Health Guide at 1-833-995-1483 to find in-network providers.
  • In Ohio, Blue Access Options PPO Network
  • Outside of Ohio, National PPO (Bluecard PPO) Network

Search for a provider at Anthem.com.

  • Under Find a Doctor:
    • Search by Plan
    • Choose Medical
    • Choose the State you are searching in
    • The type of plan is Medical (Employer-Sponsored)
    • Plan/Network is Blue Access Options PPO

Anthem Member Card

To order a new card, log into your Anthem account and do the following:

  • Select Your ID Card
  • Select the cards you wish to order
  • Select Mail. Make sure that your address is correct. If not, you must update it in Workday.
  • Click Send

Pharmacy

Anthem Rx

Contact Anthem Rx for questions concerning drug coverage, mail order, drug tiers, etc.

Anthem's pharmacy benefit manager, CarelonRx provides formulary clinical management. The following programs are included in the HDHP and PPO plans.

The Cost Relief Program works with covered members and drug manufacturers to provide copay assistance for certain specialty medications. Members taking certain specialty medications will need to call +1 (877) 638-4008 to complete the registration for Cost Relief. See the 2024 list of medications here: 2024 Cost Relief Drug List.

Sydney Health is a mobile app anthem.com gives employees a new way to focus on their health, in one convenient place. Employees can download Sydney Health and register on the app to take full advantage of their Anthem benefits in addition to a personalized, tailored experience. Members who are registered on Anthem.com can use the same username and password.
