Probationary Faculty Evaluation packets sent to the Provost for the 3rd, 4th and 5th year reviews should contain the following:

• A current CV
• Annual evaluation by the departmental P&T committee and copies of evaluations from previous years (beginning with the 2nd year)
• Annual evaluation by the Chair (as well as Program Director for joint appointments) and copies of evaluations from previous years (beginning with the 2nd year)
• Annual evaluation by the Dean (as well as Regional Campus Dean when appropriate) and copies of evaluations from previous years (beginning with the 2nd year)
• Annual evaluation by the Provost and copies of evaluations from all previous years
• Written documentation of approval for years waived (7.6.A MUPIM) or extensions granted (7.6.B MUPIM) to probationary period.

Deans submit all probationary materials (e.g. annual evaluation letters from dean, chair, committee, previous evaluations, documentation of approval for years waived or extensions granted, etc.) by April 15 to the Executive Assistant to the Provost. If the  deadline date falls on a Saturday or Sunday, the deadline date will be the following Monday. 

Note: The Provost requests that probationary faculty members begin to build their dossier for P&T starting with the 3rd year and that the dossier be included in the materials that come forward (3rd, 4th & 5th years).

• It is difficult to evaluate the faculty member's progress toward establishing a strong record of teaching effectiveness if there is no comparison with teaching evaluations in previous years (which an Annual Activities Report does not address).

Therefore, a teaching matrix needs to be submitted which includes course size, ratings for each question on the teaching evaluation form used by the department/division, number of students responding, whether course is undergraduate, graduate (simply indicating the course number is sufficient for determining the level of the course), or seminar.

• A teaching portfolio with various measures of teaching evaluation such as peer evaluations, SGID, etc. should be included and documented.

• Publications/creative activity should include quality of the journal (such things as impact, circulation, acceptance rate), prestige of the press, venue for the creative activity (juried etc.), percent effort by probationary faculty if it is a multi-authored publication, and indication if co-authors are undergraduate or graduate students.
