Helpful Information for Submitting Complaints

When submitting a complaint to the Office of Equal Employment and Opportunity (OEEO), please be sure to include as much of the information as possible. When the Notice of Investigation is issued, the written complaint will be shared with the responding party.

Include

Your Full Name:

  • Include your first and last name

Description of Incident:

  • Provide a detailed account of what happened to the best of your knowledge
  • Include the names of individuals involved and the dates of the incident(s)

Protected Classes:

Name of Respondent:

  • Identify the individual(s) your complaint is against

Type of Complaint:

  • Specify whether it involves discrimination, harassment or both

Witness Information:

  • Include the names of any witnesses, if applicable

Include Supporting Documents:

  • Attach any relevant materials such as videos, pictures, or other documents that support your complaint
