Background Check Information

Instructions to Complete the Background Check Waiver

Section 1: Personal Information

  • Name: Both first and last name.
  • Maiden Name: N/A if you do not have a maiden name.
  • Social Security Number: You have to have your SSN available in order to complete the background check (if you don't have an SSN please contact MUPD at 513-529-2226 for guidance).
  • Permanent Home Address, City, State, and Zip Code.
  • E-mail Address and Phone Number: Make sure you provide an e-mail address and phone number in case we need to contact you regarding your background check.

Section 2: Reason and Mailing Options

  • Reason for background check.
  • Have you lived in Ohio for the last 5 years? Simply check yes or no.
    • Yes, means that you have lived in Ohio consecutively for the last 5 years.
    • No, means that you have not lived in Ohio consecutively for the last 5 years.
  • Direct Copy: Depends on the reason for the background check.
  • Mailing of Results: Results can be mailed to the address of your choice or picked up in person.

Section 3: Type and Payment Method

  • Type of background check depends on the reason for the background check, consult with the requesting department or your employer or call MUPD for assistance.
  • Select a payment method (payment will not be processed/collected until you report to MUPD).

Complete waiver and payment and report to the Miami University Police Department (located next to Ditmer Parking Lot at 4945 Oxford Trenton Road / across from the horse stables) to be fingerprinted. Make sure to bring a valid driver's license with you. If you do not have a driver's license refer to “What to bring” for guidance.

Section 4: Sign and Date

This portion is completed once you arrive at the Miami University Police Department to be electronically fingerprinted.

Section 5: Account Number/Invoicing Address

This portion is completed only if a department or business is paying for the background check – leave blank if the individual is paying.

Special Note: Please do not attempt to email the form containing your SSN.

Cost for Service

  • Ohio Background Check - $38.00
  • FBI Background Check - $40.00
  • Ohio and FBI Background Check - $63.00
  • Black Ink Card - $14.00 (additional cost for multiple cards)

Turn Around Time

Results are mailed from London, Ohio to the address you provide at the time of service and typically take 7-10 business days. Results may be picked up in person at the Miami University Police Department and typically take 3-5 business days. Please note that on occasion results may be delayed and could take up to 30 days to receive.

  • Charge to Bursar (Applies to Miami University Staff and Students only)

  • Credit cards, including American Express, Discover, MasterCard, and Visa. 

If planning to pay by e check or credit card, the online payment link will be provided to customers prior to their scheduled appointment.

What to Bring

Bring a valid driver's license. If you do not have a driver's license, we will need you to bring one of the following along with a current photo ID: a passport, a state ID card, or a birth certificate. If you are under the age of 18, a parent consent form must be filled out before we can process the background check, please call to make arrangements.

Results from a Previous Background Check

Miami University Police Department does not retain copies of the results from background checks.

  • Please consult with the agency or business to whom the results were originally mailed to and request a copy for your records.
  • MU Education majors please contact the Office of Student Teaching & Field Experience at 513-529-7245.
  • Background check results are typically good for one year.

Additional Information

The FBI check can only be conducted when state legislation has been passed requiring the FBI check. The legislation must be approved by the FBI. BCI cannot process FBI background checks for individuals seeking employment, licensing, or volunteering in other states. If the reason for the background check request is not on the list of FBI reason codes on the Web Check workstation, a background check (black ink card) must be submitted to the FBI under the Freedom of Information Act.
