Miami University is committed to promoting and maintaining a safe work environment for all employees, students, and visitors. Laboratory, shop, and studio environments have unique hazards, requiring thorough and proactive oversight of occupant safety.

Injury and Illness Reporting

Occupational Injury

If you are an employee and sustain an injury or illness on any of Miami University’s campuses, complete the Employee Injury/Illness report.

Non-Occupational Injury

If you sustain an injury or illness on any of Miami University’s campuses, including students, visitors, contractors, or graduate students performing unpaid research functions, complete the Non-Occupational Injury/Illness Report.

Near-Miss Reporting

A near-miss is a potential hazard or incident that did not result in any personal injury or illness. We encourage you to submit a near-miss report to improve laboratory, studio, and shop safety at Miami University.

Shipping Dangerous Goods or Dry Ice

If you are a faculty, staff, or a researcher/graduate student planning to ship hazardous materials or dry ice through a courier service such as FedEx or UPS, a request to ship these materials must be submitted to the Environmental Health and Safety Office.

Procedure for Vacating a Laboratory

If you are planning to vacate your lab space, please follow the instructions provided by the Environmental Health and Safety Office.

I have a question regarding a hazardous material or procedure in the lab space I oversee. Who do I contact for assistance?

Please contact the Office of Research and Innovation at labsafety@MiamiOH.edu.

I would like the Office of Research and Innovation and/or the Environmental Health and Safety Office to present safety-related information in my course or to the researchers I oversee. How do I initiate this process?

Please contact the Office of Research and Innovation at labsafety@MiamiOH.edu. Please include the topics you would like for us to discuss, the dates/times/location you would like for us to present this information, and the audience type (e.g., course or research group). We will contact you for additional details.

I would like to implement training in my research lab or course. Where can I find additional information?

Please visit the Training Opportunities webpage for a summary of the courses available. Once you have reviewed this information, please contact the Office of Research and Innovation at labsafety@MiamiOH.edu for additional assistance.

An engineering control (e.g., fume hood) is not working properly in the lab space I oversee. What action do I need to take?

Please submit a service request to Physical Facilities. Discontinue use of the engineering control until it has been repaired and tested.

The biological safety cabinet in the lab space I oversee is not functioning properly. What action do I need to take?

Please contact the Environmental Health and Safety Office at ehso@MiamiOH.edu for additional assistance. Discontinue use of the biological safety cabinet until the repair has been performed and the biological safety cabinet has been recertified.

The eyewash station or the safety shower in the lab space I oversee are not functioning properly. What action do I need to take?

Please submit a service request to Physical Facilities. Discontinue use of hazardous materials requiring the presence of a safety shower or eyewash station until the repair is made and has been tested.

I need assistance selecting the appropriate personal protective equipment for experimentation that will be performed in the lab space I oversee. Who do I contact for assistance?

Please contact the Office of Research and Innovation at labsafety@MiamiOH.edu for additional assistance.

I have an idea of additional information that you should consider adding to the lab safety website. How do I submit my idea?

Please email your idea to the Office of Research and Innovation at labsafety@MiamiOH.edu. We are happy to discuss your ideas and may integrate them into our website.
